Very sad ...can lessons be learned ?

I would say leaving the safety on until you're about to shoot is good idea...

Also not having a loaded weapon when crossing an obstacle !

Absolutely, Basic safety 101 NEVER carry a loaded gun across/over/under/through an obstacle :bad:
There's room for all Gods creatures, next to the mash and gravy :)

Not being mean to the victim, but in these situations how much of the responsibility lies with yourself?...personally I wouldn't go along with someone who didn't know when to load or not...and I would check.

Safety needs to be enforced while near others even if it offends others.

Indeed. And I, for one would be happy for you to check - in fact, you wouldn't actually need to, as I would willingly show you a clear/empty gun as a matter of course.
