How would a lever-release AR prevent the bolt from returning to battery? I'm assuming some form of sear?
If so, what would stop it eventually failing?

This sounds like it's going to lead to tinfoil sears hitting the market shortly...

A long steel catch pops up as the bolt retracts, popping up in front of the bolt as it starts moving forwards again, this is then pulled out of the way by the lever release. It's rock solid.
Think of the way the sear catches the hammer, it's not too far removed I don't think.
That's just from seeing pictures of it and having it explained in a video I now can't find. I've not actually taken one apart.

I believe a number have come apart..... Just after the trigger was pulled!

I know that several were tested to destruction by the Edinburgh police.
Supposedly they managed to get them to "go fully automatic....for one shot" : work that one out!!
