Durham Police - FAC renewal form

It seems with the demise of the need for a medical (illegal requirement) they have invented a new requirement to disclose personal family details using a home-brewed form.
I utilised the Met Police's form which is an editable PDF - very easy to use - which is the legal form. Durham say they are returning it insisting that I must use the Durham form. I have declined.
I've checked Clevleand and Northumbria's form - in line with the Met's legal one.

I have to admit the medical requirement is a pet hate of mine. Absolute disgrace shooters are being asked to disclose health problems.
There has to be another way to prevent unsuitable people from becoming FAC holders.

ANNOUNCEMENT: Medical Declarations (updated 06/01/2016)

If you suffer from a relevant medical condition, you must obtain a medical report from your General Practitioner and submit this alongside your application form. This will assist the timely progress of your application, by preventing delays with the Firearms and Explosives Licensing Unit requesting a report after receiving the application.

If you have any queries and are unsure whether to obtain a medical report to submit with your application, please check with the Firearms and Explosives Licensing Unit. The original report signed by the doctor (not a copy) must be included with your renewal/grant application.

Lincolnshire Police will no longer request or pay the costs for medical reports. We are now seeking applicants to obtain medical reports only as described to determine your fitness to hold a licence. Therefore, any cost incurred must be agreed and settled between you and your General Practitioner.
As I do not have a relevant medical condition I shall refrain from ever disclosing anything on the medical side of things and therefore not incur any costs.

dodgyrog, stick to your guns (no pun intended)

Use every example you can possible find and present it to Durham and be nice and polite however mention BASC/NRA or whoever you are a member of as a side remark...

Where has this requirement come from?

Where are the "national" organisations on this?

Anyone know their position?

Besides the medical "requirement" it is a back door fees increase asking people to pay for it.

One for who you live with and another for what land you have permission for or club details with the requirement for the land owner and/or club secretary to have to sign the form.

I've also used the editable version which is exactly the same as the official HO one shown on the BASC site. I haven't had my renewal letter yet but it should be due in the next 2 weeks or so. I've also mentioned that they've got an incorrect serial number down for one of my Henry under levers, when they added the new one they put the serial number of the one I'd previously sold onto my FAC, only noticed it while I was double checking the 201 form against my rifles.

Who did you speak to at Durham, was it the lovely Laraine by any chance or did they put the refusal in writing (unlikely)?

Juli - she's pretty helpful but no doubt 'under the cosh' to coin a phrase.
I don't usually have any problems with her until she has to apply the latest 'whiz-bang' idea they come up with to divert blame from themselves.

If you'd just used the Durham ones but didn't submit the two extra sheets they probably wouldn't have even noticed!
