Spread sheet on the computer with a hard copy as back up, with targets photographed for the spread sheet.

I have a small notepad thingy (99p from Smiths) and a pencil which I keep in the cabinet. Ruled off for date, shoot, rounds fired, rounds total and notes. Combining that with my TR scorebook means I have a round count log, grouping performance log (once corrected via the graphs) and check log for the round count all in one.

TR scorebook - 1 sheet for every shoot, filed all the way back to 1983, though not well sorted. Unfortunately I do not have the score diagram for my very first TR shoot, which was kept literally on the back of a fag packet.
