Thames Valley are still showing the old forms and they are damn hard to find on their website.

Although the new forms became usable on Dec 1st the old forms can still be used up to Dec 31st.
The new forms are regarded by many as a step backwards. Every single page has to have your name printed, signed and dated on it and even twice on the declaration page!. While there are some improvements (like the removal of purchase quantities for ammunition) there was a lot more that could have been done to simplify the system.
According to BASC the forms are still a work in progress but it's highly unlikely there will be any further changes to them.

I have noticed on the new form it says to list all convictions/cautions on the old form it says list any that have been given since the last issue of FAC. So do you have to list cautions that are before the last issue of FAC now

Short answer is yes, you have to list them.
