Look carefully at the application notes. Are you talking about the doctor being a referee or supplying a medical report? A charge cannot be made to act as a referee.
No, it's an additional form that isn't part of the normal SGC/FAC application.
Instead of just ticking the box saying it's OK for the police to contact your GP they are now saying you have to hand a copy of this extra form along with the 1st page of your sgc/fac application to your GP and get them to complete it and forward it to the police, any charges made by the GP to be paid by the applicant not the police.

What if you deregistered yourself from the doctor and re register after ?
ACPO rewrote the 2013 HO guidance to say "no GP - no licence".
Dubious, because it then forces all FAC and SGC holders to use NHS medical services, irrespective of whether they want to or not.

Just had a reply back from David Ilsley at BASC,
"It’s a voluntary scheme, the Chief Constable has said so, maybe this has not filtered down to firearms licensing???
If any member has been asked or de facto forced to pay for a medical check that NOT required under the Act or Guidance please get in touch with your name and membership number and we will take this up with the constabulary."

The local BASC rep spoke to our club about this issue because some of our members are licenced by Durham his advice was that under no circumstances should you fill the form in. If you are licenced by Durham and are a member of BASC you should contact him for advice.
